Nursing Services
Our Community Nursing service provides personalised nursing care for people living with disability, helping them manage their health needs with support that respects their routines, preferences and independence.
What is Community Nursing?
Community Nursing supports people living with disability who need nursing care to manage their disability-related health needs.
Our Registered Nurses provide care, assessments and guidance based on your individual needs. We work with you and, with your consent, your health practitioners and support team to understand the care you require and how it fits into everyday life.
Our nursing service has limited capacity. Each enquiry is assessed individually, considering the care needed, the availability of appropriately skilled Registered Nurses and how soon support is required.
-
Our nursing support is tailored to your clinical needs and the care arrangements we can safely provide.
Services may include:
Enteral feeding and nutrition support, including tube feeding and PEG management
Wound care and pressure injury management, including wound and skin assessments, dressing changes, monitoring, prevention strategies and complex wound care
Continence and catheter management, including continence assessments, urinary and suprapubic catheter care, education and support strategies
Complex bowel care, including individual care planning and ongoing management
Stoma care
Respiratory and ventilation support, including tracheostomy care
Medication management, including nursing assessments, administration, monitoring, care planning and education
Subcutaneous injections, given under the skin, and other injectable medication support where clinically appropriate
Diabetes management
Seizure management
Clinical support for significant swallowing-related risks
Disability-related health monitoring
The support available will depend on your individual requirements and the qualifications, experience and clinical competencies of our available Registered Nurses.
-
A nursing assessment helps us understand your health needs, the support you currently receive and any risks that need to be considered in your care.
We develop individual nursing care plans with you, providing clear guidance for the people involved in supporting you.
This may include:
Comprehensive nursing assessments
Individual nursing care plans
Clinical risk assessments and management strategies
Education for you and the people supporting you
Ongoing clinical reviews and monitoring
Liaison with your health practitioners and other service providers
We review your care as needed so that changes in your health and support requirements can be considered together.
-
Getting started begins with a conversation about the nursing care you need, how often you need it and when you would like support to begin.
Before accepting a referral, we consider:
Whether we have Registered Nurses available with the qualifications, experience and clinical competencies relevant to your needs
The type, frequency and complexity of nursing care required
How soon care is needed and whether we can safely establish the necessary arrangements within that timeframe
Where our current nursing capacity is limited and sufficient time is available for intake, we may explore recruiting suitably qualified nursing professionals to establish a dedicated, consistent care arrangement.
We’ll discuss what is possible with you before any care arrangements are agreed.
Care built around your everyday life.
We take time to understand how you like to be supported, including your routines, communication preferences and the people you want involved. We explain your care clearly, listen to your concerns and involve you in decisions. You should have the opportunity to ask questions and understand the care you receive.
Our Approach
Confidence in the people supporting you.
Our nursing team also provides clinical governance and oversight across Flourishing Lives’ existing services.
Where our disability support workers are involved in delivering high intensity or clinical supports, our Registered Nurses provide training specific to each participant’s needs and assess workers’ competencies to deliver that support.
Ongoing clinical guidance and review help our team understand the care required and respond when a participant’s needs change.
